Ipamorelin side effects and benefits: what studies show

Ipamorelin side effects in its one human trial matched placebo, but FDA flags serious events with IV use. Claimed benefits for muscle, fat loss, and sleep are untested. Not FDA approved.

The short answer: in the only randomized trial of ipamorelin in people, adults recovering from bowel surgery had side effects at about the same rate on ipamorelin as on placebo, and the drug did not speed their recovery. [1] FDA still lists safety concerns: it states that a published study of intravenous ipamorelin identified serious adverse events, including a death, and that impurities in the peptide could trigger immune reactions. [2] The benefits ipamorelin is sold for, such as muscle, fat loss, sleep, and anti-aging, have never been tested in a human trial. It is not FDA approved.

This post separates what the human studies recorded from what is claimed, grades each claim, and explains where ipamorelin stands legally. It does not give dosing advice.

What is ipamorelin?

Ipamorelin is a lab-made peptide of five amino acids that activates the ghrelin receptor, the receptor for the body's hunger hormone. That receptor sits on the pituitary gland, and switching it on makes the pituitary release a pulse of growth hormone. Its developers described it in 1998 as the first selective growth hormone secretagogue: in pigs it released growth hormone about as well as the older peptide GHRP-6, but unlike GHRP-6 and GHRP-2 it did not raise the stress hormones ACTH and cortisol, even at doses more than 200 times higher than needed for growth hormone release. [3]

That selectivity is the reason ipamorelin is often called the cleanest peptide of its class. It was shown in animals. No published human study has confirmed it at the amounts people use.

In people, one pharmacology study gave 40 healthy men a single 15 minute intravenous infusion at one of five rates. Every rate released growth hormone, as a single pulse that peaked about 40 minutes after the start and then faded, and the drug's half-life was about 2 hours. [4] The abstract reports no side effect data, and the study did not measure IGF-1, body composition, or any clinical outcome.

What side effects did the human trial report?

The only randomized, placebo controlled trial of ipamorelin tested it for postoperative ileus, the stalled gut that can follow abdominal surgery. Ghrelin receptor activation speeds the movement of the gut, which is why the developer tried it there. [1]

The two published human studies of ipamorelin

117 adults after small or large bowel surgery, 114 analyzed; intravenous ipamorelin or placebo for up to 7 days in hospital

Any adverse event in 87.5% on ipamorelin and 94.8% on placebo; median time to a tolerated meal 25.3 versus 32.6 hours, not statistically significant

In the trial, the authors monitored adverse events and laboratory tests and concluded that ipamorelin was well tolerated, with no significant difference from placebo on the main or secondary measures of recovery. [1] Almost everyone in both groups had at least one adverse event, which is expected after major bowel surgery: most of those events come from the operation, not the drug. That is exactly why a placebo group matters, and the similar rates are the most useful safety finding in the human record.

The trial has clear limits. The authors describe it as small, with patients who had a broad range of underlying conditions. [1] It used intravenous infusions in a hospital for a week. It says little about injecting ipamorelin under the skin, at home, for months, which is how it is sold today.

Did FDA flag serious side effects?

Yes. When FDA placed ipamorelin acetate in Category 2, the list of compounding ingredients that may present significant safety risks, it gave three reasons. [2]

  • Immune reactions. Compounded ipamorelin may pose a risk of immunogenicity, meaning the immune system reacts to the product, because peptides can clump together and carry peptide related impurities.
  • Hard to characterize. Ipamorelin contains amino acids that do not occur in nature, which makes it harder to confirm what is in a vial.
  • Serious events with intravenous use. FDA states that a study published in the literature identified serious adverse events, including death, when ipamorelin was given intravenously to improve gut motility.

FDA also states that it found no safety information for some other injectable routes, so it lacks enough information to know whether the drug would cause harm by those routes. [2] The published trial abstract does not describe individual serious events, so it is not possible to say from the abstract whether any was caused by ipamorelin rather than by surgery. [1] What both sources agree on is that the human safety record is one short hospital trial.

A 2026 review of unapproved peptides used in sports medicine, ipamorelin among them, reached the same conclusion: rigorous human safety data are scarce, and there is potential for serious harm. [5]

What side effects do people report when they inject it?

No study has followed people who inject ipamorelin on their own. The closest evidence is a 2026 review of growth hormone peptides sold as research compounds, which groups ipamorelin with GHRP-2, GHRP-6, and hexarelin. Across that group, the reported adverse effects include: [6]

  • Hormone and metabolic changes, including rises in prolactin and cortisol, appetite changes, and abnormal blood sugar
  • Fluid retention
  • Muscle and joint aches
  • Injection site reactions

These are reports for the class, not measured rates for ipamorelin, and they come from products of uncertain content and strength. [6]

Hunger deserves a separate note, because ipamorelin acts on the hunger hormone's receptor. In mice, ipamorelin increased body fat by a route that did not depend on growth hormone, and secretagogues including ipamorelin raised food intake. [7] No human study has measured appetite or weight on ipamorelin.

What could long term growth hormone stimulation do?

Ipamorelin is sold to raise growth hormone repeatedly over months. No human ipamorelin study lasted longer than a week, so the best guide to long term effects is growth hormone itself.

The FDA label for somatropin, prescription growth hormone, warns about: [8]

  • Fluid retention, including swelling, joint pain, and carpal tunnel syndrome, which it says may occur frequently in adults
  • Impaired glucose tolerance and diabetes, which growth hormone may unmask
  • Tumor growth, with advice to monitor people with existing tumors
  • Raised pressure inside the skull and pancreatitis

A systematic review of growth hormone given to healthy older adults found more swelling, joint pain, carpal tunnel syndrome, and breast tissue growth in men, and a trend toward new diabetes. [9] Whether ipamorelin raises growth hormone enough, for long enough, to cause these effects has not been studied. That gap is the main safety unknown.

What are the claimed benefits of ipamorelin, and what is the evidence?

Ipamorelin is marketed for muscle, fat loss, recovery, sleep, and anti-aging. Each claim rests on the idea that more growth hormone produces those effects. Here is what has actually been tested.

The evidence behind each row:

  • Growth hormone. The single human pharmacology study showed a short growth hormone pulse after one intravenous infusion. [4] A pulse is not a clinical outcome, and no published study measured growth hormone or IGF-1 after the injections sold today.
  • Fat loss. In the mouse study, ipamorelin increased fat pads and total body fat, while growth hormone itself did not increase fat. The authors concluded that secretagogues like ipamorelin may increase body fat, possibly by increasing feeding. [7]
  • Bone. Over 15 days, ipamorelin sped bone lengthening and weight gain in adult female rats without changing IGF-1. [10]
  • Surgery recovery. The trial did not show faster recovery than placebo. [1]

PeptideAgent grades ipamorelin as having human randomized trial evidence, because that surgical trial exists. The grade describes how the evidence was gathered, not whether ipamorelin works, and the one trial was negative. The ipamorelin record lists every study with its design and size, and the growth hormone deficiency page covers the approved treatments for people who truly have low growth hormone.

No. Ipamorelin is not FDA approved for any use. [11] It was nominated for the 503A bulks list and placed in Category 2 in September 2023. FDA now lists ipamorelin acetate among 503A nominations withdrawn by the nominators, which puts it in no 503A category and outside FDA's interim policy. Ipamorelin acetate also remains in 503B Category 2, added September 29, 2023. [2] It is not on the 503A bulks list, so neither 503A pharmacies nor 503B outsourcing facilities have a federal basis to compound it.

Products labeled research use only are unapproved drugs when sold for human use, as FDA stated in an August 2026 warning letter to an online seller of research-use-only peptides. [12] Their identity, purity, and sterility are not verified. The ipamorelin regulatory tracker logs each dated status change, and the ipamorelin cost page explains why there is no lawful price to list.

Is ipamorelin allowed in sport?

No. WADA names ipamorelin in section S2 of its Prohibited List, among growth hormone secretagogues, and it is banned at all times. [13] Anti-doping laboratories have published methods to detect ipamorelin and its breakdown products in urine. [14]

What about ipamorelin combined with CJC-1295?

Ipamorelin is most often sold blended with CJC-1295, a long acting growth hormone releasing hormone analog. The pair has never been tested together in a human trial. Side effects of CJC-1295 alone and of the combination are covered in CJC-1295 side effects, and the pairing has its own CJC-1295 and ipamorelin blend page. Clinics also pair ipamorelin with sermorelin and tesamorelin; see the sermorelin and ipamorelin and tesamorelin and ipamorelin blend pages.

For side by side comparisons, see CJC-1295 vs ipamorelin, sermorelin vs ipamorelin, tesamorelin vs ipamorelin, ipamorelin vs GHRP-6, and ipamorelin vs MK-677. All three growth hormone peptides are compared in one table in tesamorelin vs sermorelin vs ipamorelin.

Talk to a clinician first

If you have used ipamorelin and notice swelling, numb or tingling hands, joint pain, constant hunger, or signs of high blood sugar such as thirst and frequent urination, talk to a licensed clinician and bring the product label. If the real concern is low growth hormone, an endocrinologist can test for it and discuss the approved options. For the lawful routes to peptides that do have one, see how to get peptides prescribed.

What's actually offered, and what that means for you

Many clinics offer ipamorelin anyway. FDA has also found compounding pharmacies and an online seller offering it. [15] [16] [17] [18]

What that means for you

  • A pharmacy that compounds ipamorelin is taking a regulatory risk: it has no federal basis to do so, and FDA has sent warning letters to compounders for the same thing. [19] [20]
  • No FDA oversight of the specific product: FDA does not check a compounded drug's safety, effectiveness, or quality before it is sold. [21]
  • State boards can act. On August 19, 2026, Mississippi's medical, nursing, and pharmacy boards said no licensed provider may prescribe, recommend, supply, or administer a peptide that is not FDA approved, and that a consent form does not remove the provider's liability. [22]
  • Louisiana acted by statute: Act 374, in effect August 1, 2026, bars its licensing boards from prohibiting peptides only when they come from a pharmacy or outsourcing facility that complies with federal compounding law. It does not make any peptide eligible to compound. [23] [24]

If you're going ahead anyway

Each step is on the ipamorelin page: check the pharmacy's license, what a legitimate prescription and consult look like, why this is safer than a research-use-only product, and questions to ask. These steps lower some risks. They do not make the product safe, and they do not give a pharmacy a federal basis to compound ipamorelin, with or without a prescription. [11]

Decisions about starting, stopping, or combining any treatment belong with a licensed clinician who knows your history.

Frequently asked questions

What are the side effects of ipamorelin?

In its only randomized human trial, adults given intravenous ipamorelin after bowel surgery had adverse events at a rate similar to placebo (87.5% versus 94.8%), and the authors called it well tolerated. FDA separately states that a published study of intravenous ipamorelin for gut motility identified serious adverse events, including a death, and it flags immunogenicity risk from impurities. Injected use outside trials has not been studied. [1] [2]

What are the benefits of ipamorelin?

None has been shown in people. A single intravenous infusion released growth hormone in healthy men, and in animals it increased bone growth. No human trial has tested it for muscle, fat loss, sleep, recovery, or aging, and its one randomized trial, for bowel recovery after surgery, found no benefit over placebo. [1] [4] [10]

Does ipamorelin cause weight gain or hunger?

It acts on the receptor for ghrelin, the hunger hormone. In mice, ipamorelin increased body fat by a route that did not depend on growth hormone, and secretagogues including ipamorelin raised food intake. Appetite changes are also among the effects reported by people who self-inject growth hormone peptides. No human trial has measured it. [6] [7]

Is ipamorelin FDA approved?

No. It is not FDA approved for any use. FDA now lists ipamorelin acetate among 503A nominations withdrawn by the nominators, and ipamorelin acetate also remains in 503B Category 2, added September 29, 2023. It is not on the 503A bulks list. Products labeled research use only are unapproved drugs when sold for human use. [2] [11] [12]

Is ipamorelin banned in sport?

Yes. WADA names ipamorelin in section S2 among growth hormone secretagogues, prohibited at all times, and anti-doping laboratories have published methods to detect it and its metabolites in urine. [13] [14]
