Selank nasal spray: the evidence, and combining it with semax

The short answer: selank nasal spray is the route selank was studied in, and the studies are small Russian trials in anxiety that were not placebo controlled. [4] Combining it with semax, a popular pairing sold as "semax and selank nasal spray", has never been tested in any trial, human or animal. Selank also has no lawful access path in the United States: it is not approved, not on the 503A bulks list, and was not part of the July 2026 advisory committee recommendation. [15]

What is selank nasal spray?

Selank is a seven amino acid analog of tuftsin, an immune peptide fragment, with a Pro-Gly-Pro tail added for stability. It was developed in Moscow and is used in Russia as nasal drops for anxiety. Early work from the developers proposed that it acts partly by slowing the breakdown of enkephalins, the body's own opioid peptides: selank inhibited enkephalin-degrading enzymes in human plasma in the lab. [1] Later rat work linked it to changes in genes involved in GABA signaling, the same system benzodiazepines act on. [7]

Products sold online as "selank spray" or "selank nasal spray" are research-labeled liquids, not the Russian product. They are not approved in the United States in any form.

What did the intranasal selank studies find?

Three Russian clinical studies carry most of the weight.

• Selank vs medazepam (2008). Sixty-two patients with generalized anxiety disorder or neurasthenia were studied, 30 on selank and the rest on the benzodiazepine medazepam. Anxiety improved similarly in both groups, and selank also showed anti-fatigue and mildly stimulating effects. The authors linked the response to changes in blood enkephalin measures. [4]
• Selank vs phenazepam (2014). In 60 patients with phobic anxiety and somatoform disorders, the authors reported a pronounced anxiolytic effect and a mild nootropic effect with selank, lasting about a week after the last dose. [5]
• Selank added to phenazepam (2015). Seventy patients received phenazepam alone (30) or phenazepam plus selank (40). The combination reached its effect sooner and reduced several benzodiazepine side effects, including sedation and memory problems. [6]

These are useful signals, but they are weak evidence. The trials were small, open or single blind, published in Russian, and run within the network that developed selank. None used a placebo arm in a way described in an indexed English-language report. Does the route matter? Nasal vs injected selank

Searches for "selank spray" often ask whether the nose beats a needle. The only direct comparison is in mice.

When researchers gave selank intranasally or by intraperitoneal injection for five days, both routes reduced anxiety-like behavior, but only in a naturally anxious mouse strain, not in a calmer one. The routes also left different fingerprints in the brain: injection increased GABA receptor binding in the frontal cortex, while the nasal route increased NMDA receptor binding in the hippocampus. [8] The authors put this down to differences in how the drug is absorbed and broken down. No human study has compared the two routes, and injected selank has no published human data.

Semax and selank nasal spray: has the combination been tested?

No. We found no human or animal study that gave semax and selank together, and no study that measured whether combining them changes benefit or risk. People who search "semax selank nasal spray" or "selank and semax nasal spray" are mostly looking at blended products that have never been evaluated.

What exists is research on the two peptides side by side, each tested on its own:

• Shared enzyme effect. In human serum in the lab, both semax and selank inhibited enkephalin-degrading enzymes, with semax slightly more potent. [2] If each slows the same enzymes, combining them could add to that effect in ways nobody has measured.
• Rat behavior. In rats with a chemically induced Parkinson-like injury, neither peptide changed motor activity, and selank alone reduced anxiety-like behavior. The peptides were given separately. [9]
• Brain imaging. A resting-state imaging study compared semax, selank and placebo and reported both shared and distinct effects on connectivity between the right amygdala and the temporal cortex. Again, each peptide was given alone. [10]
• Different main targets. Semax is studied mainly for its effect on BDNF, a nerve growth factor, in the rat hippocampus. [3] Selank is studied mainly for GABA-related effects. [7]

The honest summary is that the combination is a marketing idea, not a studied therapy. Overlapping proposed mechanisms are a reason to expect additive effects, including unwanted ones, not evidence that the pair is safe or better. Is selank nasal spray legal?

Selank has no lawful route in the United States today.

1. FDA placed selank on its 503A Category 2 list of bulk substances that may present significant safety risks in 2023. FDA's current page lists selank acetate as a withdrawn nomination, which means it is off Category 2, not approved for compounding. [13]
2. It is not on the 503A bulks list, so a 503A pharmacy has no federal basis to compound it from bulk. [14]
3. It was not among the peptides the Pharmacy Compounding Advisory Committee recommended on July 23 to 24, 2026. [15] Semax was, which is why the two now sit on different legal tracks.

Side effects, safety and product quality

The Russian studies reported that selank was well tolerated compared with benzodiazepines. [5] Those were short courses in small groups, so rare or long-term harms would not show up. A 2021 review of GABA-acting drugs described selank as a poorly studied Russian drug sold to US consumers as a dietary supplement, and warned that GABA-acting agents need careful study of abuse potential before public access. [12] Questions about dependence and stopping are covered in our selank withdrawal explainer.

Quality is a separate problem. A Belgian medicines laboratory that analyzed seized preparations found selank and semax, noted that they had not completed clinical trials, and found them sold freely online as injectable powders and nasal sprays. [11] Modified versions marketed as stronger forms are covered in our post on N-acetyl semax and selank amidate.

The bottom line

Selank nasal spray rests on a few small Russian anxiety studies with no placebo arm. Semax and selank nasal spray together rests on nothing: no trial has tested the pair.
