Verdict: favors Semax

Semax and Noopept are both Russian nootropics with only small open label human studies, but they reach users very differently. Semax is an intranasal peptide that came off FDA Category 2 in April 2026 and was recommended for the 503A bulks list, so it may be prescribed and compounded. Noopept is an oral piracetam relative that has never been nominated for compounding and is sold in the US as a supplement ingredient, which is not a lawful use.

Which is better, Semax or Noopept?

The evidence is a tie: both are graded human observational from open Russian studies without placebo control (semax in stroke, Noopept in mild cognitive impairment after stroke or brain injury). Regulatory status breaks it. CompoundingSemax was placed on the FDA 503A Category 2 list (substances with significant safety risks) in 2023 and removed from it on April 15, 2026. On July 23 to 24, 2026 the Pharmacy Compounding Advisory Committee recommended adding semax to the 503A bulks list. As of the last verification date FDA has not published a final rule, so compounding pharmacies and state boards vary in whether they will fill it. It is not an FDA approved drug and is not eligible for 503B outsourcing. It is a registered prescription drug in Russia.Noopept has never been nominated to or reviewed for the FDA 503A or 503B bulks lists and does not appear on the FDA Category 2 page (current as of April 22, 2026). Frequently asked questions

Is semax or Noopept better for memory?

Neither has a placebo controlled trial for memory. Noopept's human data are in mild cognitive impairment after stroke or brain injury (better MMSE scores versus untreated controls in an open study of 60 patients); semax's are in stroke recovery. Evidence in healthy adults is absent for both. [1] [2] [3]

Is Noopept a peptide?

Not strictly. It is a small synthetic dipeptide-like molecule related to piracetam that is metabolized to the natural dipeptide cycloprolylglycine. Semax is a true seven amino acid peptide. [7] [8]

Is it legal to buy Noopept as a supplement?

It is sold that way, but a synthetic drug substance approved as a medicine in Russia does not meet the definition of a dietary ingredient, so the sale is not an approved use and products are not verified for identity or purity. Noopept also cannot be compounded, because it has never been nominated for the bulks lists. [5] [6]

Can a US pharmacy compound semax?

Not from bulk today. Semax came off FDA Category 2 on April 15, 2026 and PCAC recommended it for the 503A bulks list in July 2026, but the recommendation is advisory: until FDA publishes a final rule it is not on the bulks list, a 503A pharmacy has no federal basis to compound it from bulk, and state boards cannot authorize what federal law does not. [4] [11]

Are either banned in sport?

Neither is named on the WADA Prohibited List. Tested athletes should confirm with their anti-doping organization, because unapproved substances can fall under S0. [12]
