MOTS-c for weight loss: what mouse and human studies show

MOTS-c prevented diet-induced obesity in mice, but no human trial has tested it for weight loss. What the mouse data show, what human blood studies found, and where it stands legally.

The short answer: MOTS-c has not been shown to cause weight loss in people. In mice on a high-fat diet, MOTS-c prevented diet-induced obesity and insulin resistance, and that single 2015 study is the source of most weight loss claims. [1] No published human trial has given MOTS-c and measured body weight or fat. The first randomized, placebo-controlled human trial we found was registered in 2026, is still recruiting, and measures insulin sensitivity rather than weight. [3]

This page walks through what the mouse studies found, what the human blood studies can and cannot tell you, and where MOTS-c stands legally. It does not give dosing, and no dosing for MOTS-c has been established in people.

Does MOTS-c help with weight loss?

In mice, yes. MOTS-c is a 16 amino acid peptide encoded in mitochondrial DNA. The 2015 study that named it found that its main target appears to be skeletal muscle, where it inhibits the folate cycle and activates AMPK, the cell's energy sensor. Treated mice were protected from both age-related and high-fat-diet-induced insulin resistance, and from diet-induced obesity. [1]

In people, the answer is unknown. Read the mouse result carefully: it shows that MOTS-c prevented weight gain in animals fed a fattening diet. That is a different question from whether it helps a person who already has obesity lose weight, and no human study has answered either one.

Our evidence grade for MOTS-c is animal only, the lowest grade that still has controlled experiments behind it. The obesity condition page ranks it below every approved GLP-1 medicine, each of which has large randomized trials with weight as the primary endpoint.

Does MOTS-c burn fat? What human studies measured

The human MOTS-c literature measures blood levels. It does not test treatment. Three findings come up most often.

  • Levels are higher, not lower, in obesity. A 2026 study compared 32 adults with obesity scheduled for bariatric surgery with 22 lean adults. Circulating MOTS-c was higher in the obesity group, rose sharply once insulin resistance passed a threshold, and did not change 6 months after surgery despite large drops in BMI. The authors suggest MOTS-c may be a compensatory response to metabolic stress. [4]
  • Levels track with abdominal and liver fat. In 125 adults without diabetes, plasma MOTS-c was positively associated with android (abdominal) fat and liver fat. [5]
  • A gene variant is linked to diabetes. A mitochondrial DNA variant that changes one amino acid in MOTS-c was associated with type 2 diabetes in Japanese men, and the variant peptide had weaker metabolic activity in cells. [6]

None of these studies gave anyone MOTS-c. A higher blood level in people with more fat does not show that adding more MOTS-c burns fat; it could equally be the body's response to insulin resistance. The insulin resistance page covers the treatments that have been tested in people.

Does MOTS-c give you energy?

The energy claim comes from a 2021 study. MOTS-c treatment improved running performance in young, middle-aged, and old mice, and treatment started late in life improved physical capacity and some healthspan measures. [2] The same paper sampled 10 healthy young men and found that exercise raised MOTS-c in their muscle and blood. [2]

So exercise increases MOTS-c in people, and MOTS-c increases exercise capacity in mice. What has not been shown is the step in between: that giving MOTS-c to a person improves energy, fatigue, or endurance. A 2023 review noted that no effective method of applying MOTS-c in the clinic has been developed. [7]

How long does MOTS-c take to work?

There is no human answer, because there are no human results. The mouse studies measured effects over weeks of treatment, and mouse timelines do not transfer to people. [1] [2] The registered phase 2a trial will randomize about 120 adults with prediabetes and overweight or obesity to MOTS-c or placebo for 12 weeks, with a change in an oral glucose tolerance test measure of insulin sensitivity as the primary outcome, plus safety through week 16. [3] Until that trial reports, any timeline or "results" claim is anecdotal.

MOTS-c results: why before and after claims do not count

Online MOTS-c results are usually self-reports from people who also changed diet, exercise, or other medicines. Without a control group, a report cannot separate the peptide from everything else. The only controlled results for MOTS-c are in animals. [1] [2]

MOTS-c nasal spray, injection, and supplement forms

Every MOTS-c treatment study we found used injection in mice, and the registered human trial uses subcutaneous injection. [1] [3] We found no published study of MOTS-c as a nasal spray, capsule, or oral supplement in people or animals, so there is no evidence on whether those forms deliver the peptide at all. Products sold in those forms are untested on that question.

Stacking MOTS-c with other peptides

Some people ask about combining MOTS-c with GLP-1 drugs or with retatrutide. No trial has tested MOTS-c with any other drug. MOTS-c is not an FDA approved drug. On July 23 to 24, 2026, FDA's Pharmacy Compounding Advisory Committee recommended adding it to the 503A bulks list. [8] That vote is advisory. Until FDA publishes a final rule, MOTS-c is not on the 503A bulks list, so a compounding pharmacy has no federal basis to make it from bulk. [9] Products labeled for research use are not lawful for human use and are not verified for identity or purity.

For tested athletes, MOTS-c is prohibited at all times under WADA section S4.4, which names it as a metabolic modulator. [10]

What would change the evidence grade

Our grade for MOTS-c would move up only when a controlled human trial reports results. The registered phase 2a study is the first candidate: it is randomized, double-blind, and placebo-controlled, and it plans to report insulin sensitivity, HbA1c, fasting glucose, and anti-drug antibodies. [3] Even a positive result would answer a metabolic question, not a weight loss one, because body weight is not a listed outcome. A weight loss claim would need a trial with weight or fat mass as a primary endpoint, run long enough to see a difference, and reported with its side effects.
